Message from the Head Nurse

Dear Parents and Guardians,

Welcome! My name is Fallon Nolden, and I am the Head Nurse for Cahokia Unit School District #187. With over 20 years of healthcare experience, my top priority is the safety and well-being of our students.

To help us care for your child during illness or emergencies, please keep your contact information updated with the school office and complete and return the required forms to the school nurse.

It is important to notify the school office of any changes to phone numbers or emergency contacts throughout the school year. If your child becomes ill and cannot return to class, the parent or guardian will be contacted first, followed by the emergency contacts listed on the form. In the event of a medical emergency, Emergency Medical Services (EMS) will be called if needed.

HEALTH HISTORY FORM - Please complete and sign the attached Health History Form. This information helps the school nurse meet your child’s health needs while at school.

If your child has any medical conditions, allergies, or care plans, please include those forms or any medical documentation that will help us provide safe and appropriate care.
